Approximately one person in 10 taking diet drug Acomplia (rimonabant) experiences psychiatric side effects and about one person in 100 experiences suicidal thoughts, according to U.K. regulators.

Of some 41,000 patients treated with rimonabant since it was launched in the U.K. in June 2006, 364 “psychiatric reactions have been reported,” the Department of Health’s Medicines and Healthcare Products Regulatory Agency (MHRA) said on July 19th.

“Amongst these, there have been 48 reports of depression, 16 reports of suicidal thoughts and one report of self-injury,” the regulatory agency said.

The MHRA said evidence suggested one person in 10 taking Acomplia may develop psychiatric side effects, and “approximately one patient in every 100 may experience suicidal thoughts.”

The MHRA advised: “If you start to experience symptoms of depression while taking Acomplia, or if you are currently being treated with antidepressants, consult your doctor.

“If you have had depression in the past but feel well on Acomplia, you should continue treatment with Acomplia and discuss your treatment with your doctor at your next routine appointment.”

The European Medicines Agency, however, took a more aggressive approach, deciding that Acomplia can remain on the market but with tougher warnings against its use by persons with major depression or anyone on an antidepressant.

The EMEA’s Committee for Medicinal Products for Human Use (CHMP) said “too many patients are taking Acomplia at the same time as antidepressants, even though Acomplia is not recommended for use in patients also taking antidepressants.”

While the CHMP said “Acomplia must no longer be used in these patients,” it concluded “that the benefits of Acomplia continue to outweigh its risks, except in patients with ongoing major depression or taking antidepressants.”

The European Medicines Agency (EMEA) recommended contraindicating Acomplia (rimonabant) from sanofi-aventis, in patients with ongoing major depression or who are being treated with antidepressants, because of the risk of psychiatric side effects. Doctors in the EU have already been warned about this since June 2006 but the Agency’s Committee for Medicinal Products for Human Use (CHMP) has now recommended upgrading this warning.

A new animal study on rimonabant (Acomplia / Zimulti) has identified yet one more possible health benefit for this controversial diet drug: it appears to reduce obesity-related liver damage that can lead to cirhossis of the liver.

A team of researchers led by Mohammed Bensaid of Sanofi-Aventis, the company that has developed rimonabant, reported on these latest developments in the July 2007 issue of Hepatology, the official journal of the American Association for the Study of Liver Diseases.

In their study involving liver function in obese rats, the researchers said they found that rimonabant reduced markers of liver damage, decreased levels of pro-inflammatory proteins, and improved lipid profiles.

Male obese rats were given rimonabant orally daily for 8 weeks and had their food intake monitored; control animals received the same amount of food as those receiving rimonabant.

The researchers reported the results showed treatment with rimonabant reduced liver enlargement, completely abolished hepatic steatosis, and decreased blood levels of enzyme markers that indicate liver damage.

It also strongly reduced levels of hepatic TNFa, a pro-inflammatory protein thought to induce insulin resistance in the liver and be involved in the progression of steatosis to hepatic fibrosis and cirrhosis.

“Our hypothesis is that the multi-protective effects of rimonabant may be mediated for a large part by both the reduction in pro-inflammatory cytokines such as TNFa and the increase in anti-inflammatory and protective cytokines or hormones such as adiponectin,” the researchers said.

“This suggests a potential clinical application for this CB1 receptor antagonist in the treatment of liver diseases associated with obesity and the metabolic syndrome,” they added.

The researchers also emphasized that these results on Acomplia diet drug were not (or were only slightly observed) in the control animals eating the same diet but not given rimonabant, demonstrating the beneficial effects of the drug compared to diet alone.

Acomplia, or rimonabant, is an experimental drug developed by the French pharmaceutical firm Sanofi-Aventis primarily as a diet pill, but the drug works differently that most diet pills in that it blocks the brain’s reward system.

It works by blocking the connection between a pleasure-seeking activity, like smoking or overeating, and the reward “feeling” it causes in the brain. The human body contains endocannabinoids, marijuana-like substances that can stimulate the brain’s reward system.

Acomplia blocks the effect of the natural endocannabinoids by keeping them from attaching to the brain cells they usually stimulate, scientists say.

In March of this year, Acomplia gained a great deal of attention when two research studies demonstrated that it could not only help people lose weight, but quit smoking also. Rimonabant, launched in the UK last summer, has been shown to aid weight loss by reducing appetite.

But a committee advising the US Food and Drug Administration (FDA) voted to ban the pill, saying the manufacturers had failed to show it was safe.

European regulators will review the drug’s safety at an upcoming meeting.

Around 37,000 Britons have been prescribed rimonabant which can help users lose up to 10% of their body weight.

European Regulators Decide Diet Drug Rimonabant Should Remain on the Market

 

       

While diet drug rimonabant may double your risk of depression, its benefits “continue to outweigh its risks” for overweight and obese individuals who are not taking antidepressants, European drug regulators concluded on July 19th.

But in deciding to let Acomplia remain on the market in Europe, the regulators expressed concern that “too many patients are taking Acomplia at the same time as antidepressants,” and issued a tougher warning to doctors to stop prescribing it to patients on antidepressants or suffering major depression.

The European Medicines Agency (EMEA) issued their decision after reviewing safety data that led a U.S. FDA advisory panel last month to unanimously recommend against allowing rimonabant (also known as Zimulti) to be prescribed in the United States.

The EMEA’s Committee for Medicinal Products for Human Use (CHMP) said it asked Sanofi-Aventis following the FDA advisory panel meeting in June 2007 to “submit all available information on the psychiatric side effects of Acomplia.”

In assessing the data at its just concluded July meeting, the CHMP said it concluded “that the benefits of Acomplia continue to outweigh its risks, except in patients with ongoing major depression or taking antidepressants.”

Acomplia is approved in Europe as an adjunct to diet and exercise for the treatment of obese or overweight adult patients.

The EMEA said that while doctors were warned when rimonabant was approved a year ago that they ” should not prescribe Acomplia in patients with uncontrolled serious psychiatric conditions such as major depression,” the warning will now be upgraded.
